My ex sliced both his nipples off. Before he did that he like piercing them with needles. He said he enjoyed it. He wanted me to try it and I would not. I don’t find these things to be enjoyable or erotic just sick.

Is he an emo? That sounds like an emo to me because he like hurting himself but I never get what the emo do to themselves anyway.
Thats masochism. Masochism is being turned on by hurting/humiliating yourself, or someone else hurting or humiliating you. Well actually true masochism is being unable to be turned on without that. But it is more loosely used as just the enjoyment of that.
Sadism is the compliment to that…..a sadist is turned on by hurting or humiliating someone else, or by someone elses pain or humiliation. Also, true sadisim is not being able to be turned on without that, but is most often used as just the enjoyment of it.
The combination of the two is were Sadomasichism (the S&M end of BDSM) comes from….is agreement between 2 people, one that enjoys being hurt or humilated, the other which enjoys to hurt or humiliate
Edit to add:
Yes if he can not get an errection without doing things like that then he is a true masochist. He could see a therapist. Its not really known why people end up with fetishes (thats what masochism is a type of fettish). Same with foot/shoe fettish, crossdressing fetish……there are lots of fetishes that most people do look at as being strange, but for the most part they are harmless….but cause of them is not really known. But the thing with fetishes, if it doesnt bother him, and he is not at a risk of really harming himself or someone else, then there is no real reason to try to help it. However, if you are not at least somewhat compatable with his fetish or tolerent of it, there really is no way to make a relationship work.
